The Environmental Health Division maintains its most commonly requested files in electronic format on the internet as a service to the public. Electronic files may be viewed, downloaded or printed from this site free of charge. While the Environmental Health Division strives to maintain accurate electronic copies of records; it has not verified that every electronic file is complete.

Please enter either the Name, ID, Address, or Period for the property you want to search in the Enter Search Filter box below and click Go and you'll be taken to your search results.
Select your choice from the results and then click View (on the right side) to view the associated PDF document.
For Advanced Searches, please click on the
link below and provide your information.
Advanced search will allow searches in multiple fields, i.e. Name, ID, etc. By clicking the green +Add button, you can add more search criteria to refine the desired results. Make your additional refinements then click the Apply button to view your results.
